The injury to CM Punk's nose happened when he entered the ring and Randy Orton broke away from Nexus and lunged into Punk. Orton inadvertently headbutted Punk, causing the injury. Punk covered up and there was already blood on his body as the rest of Nexus came and beat Orton down.
